I've gone back and forth on this over the years. Sometimes I send a quick generic request, other times I'll add a short note if it's someone I genuinely want to connect with.
 
A short personal note usually works better it shows there’s an actual reason behind the connection.

It depends on who I'm reaching out to. For someone I genuinely want to connect with, I'll write a short personal note. For general networking, I usually keep it simple.
 
When possible, always. And it's best done with people who are already second connections. Otherwise the connect rate goes way down.
 
Blank for volume, personalized notes only for actual leads. Nobody reads the generic ones anyway.
 
Same here honestly. If it's a cold outreach for business I keep it short and generic just to save time, but if I actually looked at their profile and it's relevant to what I do I'll write something specific. Generic notes for everyone just feels lazy after a while.
 
A brief note about your common interest or any obvious reason to connect is always perceived as being more authentic. I prefer personalizing my requests when there is anything relevant that I can mention and not in all cases where I send a request.
