I don't on high traffic sites, smaller sites I refresh the look and even change the theme

I never change the colour scheme, although I will update a site's overall look if I think it needs an update.

If the site was looking pretty boring, then I would consider changing it.

Yes, I did it sometimes when it's needed to redesign. Mainly to impressing my visitors.

It's a good idea to refresh but not significantly change. If you have a lot of visitors that like your's website view then big changes can scare them. What about mini vote on the site on this theme?

It's always worth considering alterations. I generally try and identify specific elements that could be improved, split test and then alter based on the results of a test - rather than just make subjective changes based on a gut feeling.
