Can anyone tell me what theme this is? Or one that looks really similar to it?

Also, can anyone point me to a good plugin that give me a custom form like the one seen here?

I'm pretty much sure this is hard PHP coding and not a commercial theme.
What are you looking for? Wordpress, drupal?

Yeah, it's not CMS (Content managment system) like Wordpress, Jumla, etc.

It's custom designed on Dreamweaver probably.

wordpress preferably although i can use joomla.

If you are looking for Real Estate theme, I wolud recommend you Studiopress' Agentpress theme:

Not as minimalistic as your referal theme, but it's worthy!

Thanks for the suggestion, but a lot of times just because a WP theme is designed for a niche doesn't make it appropriate.

In this instance, the minimalism is key

Check out gravityform, it can do the exact same form and send the data to your email in a format you want.

It's not the exact same thing but I think with a little tweaking it would work just fine.
