Do synonyms count as duplicate content?

I have a website in the medical niche. I have around 100 articles on my site so far but a lot of them are as follows: "keyword strain", "keyword tear", "keyword treatment" etc. Each one of those phrases are its own article. Should I consolidate these into one big article? Each one of those phrases has very different search volumes and they each have varying advice on what to do, i'm just worried it might be duplicate content because while each article contains different information, there is obviously some overlap.

Also I have some articles that are as follows: "medical term decay", "general synonym decay". I know a lot of people might not know the medical term so I write another unique article using the general term. example would be "tooth decay treatment" and "enamel decay treatment". Some people might not know what enamel is but they certainly know what teeth are.

If you are using Google keyword planner and they are separate keywords then you
have no problem at all with duplicate content. Duplicate content comes from copying other
websites and/or posting the same article on your site over and over.
 
Yeah synonyms won't hurt you. However, if you steal someone's content and just swipe out of few words with the synonym versions, then that might be a different story. I would just be safe and always have unique content. It's easy to rewrite or put a different spin on a subject. Inject your own thoughts and opinions into your writing, which will always make it unique.
 
Given that most spinners spin mostly using synonyms, and still manage to create reasonably (>60%) unique content, I don't see that you'd have an issue.

And in any case, big is not a duplicate of large so I don't see how you'd have an issue.

The problem with spinners is that you get a completely unreadable. For a spinner to work you should use heavy spun and I suggest manual spun. Doing this will get you 60% diferent readable content.
