Say you get denied by an affiliate program, can you reapply with the same information successfully with a better approach? Or do you need to come in with a new email, new name, new phone number, bla bla bla. Not banned, but just denied at application.

one affiliate network denied my application too but when i contacted them on aim,the rep said it was denied by an automated system and he did not even review it.so he asked me a few questions and then approved my application in 15 minutes while he was chatting with me.
so moral of the story is contact them via yahoo or aim.

MarketLeverage rejected my application and said I can reapply after 6 months.

I'm not going to reapply though..I'm going to stick with neverblue, hydra, and azoogle who were happy to approve me the first place.

CPAempire rejected me too with the reason they couldn't reach me on the phone (I'm from Australia and never get their calls) and Copeac hasn't responded after a week. It is just me but I'm not going to make any single cent for the networks that turn down my first application.

There are lots of good networks out there...

I would say call them and ask . Once you talk to them it builds repoir. If it doesn't work set i up under differnet info. Company or personal info...
