I used to have a HTML page that got crazy high ad clicks but unfortunately no idea how to turn it into WP. I find that the CTR varies a great deal depending on the traffic and how people find your site... I think that may be more important than the design per se. I did get a nice CTR for one of my pages with the CTR theme though other pages had miserable CTRs... So I really have a feeling the visitors mean more than the design.
yes - my advice, chose a popular and good looking theme related to you niche and implement the ads fitting for your content and considering the adsense heatmap http://www.vaughns-1-pagers.com/internet/google-heatmap/google-heat-map-3page.gif
Yes there are good pre-optimized themes but if you want the best CTR you are blending your ads perfectly by yourself for your site. CTR above 10% are easily manageable(depends on niche). I also recommend to read some stuff about the visual perception of the human eye.
i can't say it's 100% because of this theme, I don't know why my adsense has ran well for 1 year until i used this theme. I think this theme comes with invalid clicks, but i said im not sure.
