Crackrevenue registration problems

Im start reading this forum about 3 month's ago, now i decide with method is best for me.
Im from Poland and I want to join to Crackrevenue program but my language is bad and I dont understand some registration things.
I have registred 2 domains and i want to use it with Adrian112's method: REUPLOAD - 70$+ per day Porn Uploading system
I dont understand what i must fill in registration form on CAMPAIGN STEP.
