CPA Submit hidden

I'm looking for someone to help me create a landing page that allows the user to submit their email, but for the page to look like a popular site rather than the regular CPA ad. Blank the referrers and pass the information on to the CPA networks. I have plenty of traffic and CPA accounts already setup.

What budget are we talking about here?

No real budget. But with the amount of traffic I can push thru it would be best for the person to work on a % deal.
