I got approved on peerfly and maxbounty on the same day. Even if you get rejected, just give a call and be bold while talking ask what is the reason to decline the application and be true with the traffic source you use, they will approve.
As per my knowledge, they are most happy with PPC traffic from bing, FB and G Ads and not too much interested in email traffic, but they are okay with search traffic, if your site has good number of visitors and is related to the niche what you tell them you're going to promote.
BTW, I'm from India and it's real easy to get approved, if you have already worked with other networks.

Yes, most of the times approval is just a call away. Only think they are from you the quality of your traffic. So, if your doing SEO and have good website with target traffic or if you're doing PPC on a reputed network, think you got approved.
Call them and talk to them, they won't ask how good is your English while talking to them, cos many non-English publishers are working with them and they should be good with you. Just ask them to go slow with you while speaking, if you don't understand their fast speech.

Somehow but once you will clear when you call and ask the reason behind disapproval.
1. Make a good website
- themeforest got some eye catching theme, if you are on tight budget, get the shared one around this forum or mafiashared,
- Get a nice logo, fiverr or design by yourself http://www.abstractfonts.com/ get some really nice font there for your logo.
2. Post some content. Original or scrape one, doesnt matter.
3. Use your domain email, dont use yahoo/gmail.
4. Put some fake banner on your website. So its looks like you are already working with another advertiser.

I can say that the CPA companys can be a little hesitant if the methods of promotion and your short biography on Internet marketing don't seem intact. However, I used to help an Asian guy make applications, he simply would share his application details with me and I would call the company as am interested in working soon. If questions are answered well, they never hesitated to forward the application and grant me an account.

It's true that some networks may be hesitant with Indian/Asian countries but there are thousands and thousands of solid publishers from those regions so it's best to just be as accurate as possible on your application and include detailed information of your promotion sources & methods as that is the most important part for our application process. I'm asian,but we also could find CPA network,it is also depend on your company and project.
