CPA Idea - Need Help with Automatic Code Deliviery

Hello all,

I got idea to make website with e.g Itunes GC for Free ( I mean for Filling Survey) but I have question related Code Deliviery. How I can make something like this on wordpress :
User Click "Get Code" -> Enter Email -> content locker pop - > he/she fill a survery -> code is automatically delievered to his/her email address.

Anybody can help or make a plugin to do this ?
