Its easier to just google for privacy policy generator. You input your info and it places it in all the spots needed as it generates the full policies for ya, takes like 2 seconds and you have one for your site with all your info placed throughout. That's how I always do it anyways when I'm not using custom solutions that already populate that along with other stuff at setup.

Yes, I think it is a copyright infringement. Just rewrite it to skip any problem. And don't forget about Google.
