Quite a lot of factors play into that. Some of the few are, suspicious login or account activity, sudden spike in activity, or poorly configured connection environment. There's a lot more but those are some you can immediately check or address.

The phone verification is only due to bad IP or spammed ip, and your automation. Other several reason Your useragents are not matching, and your anti detect browser is probably leaking too much info. Your automation is not being helpful enough, its not mimicking a normal human behaviour.
