You can try accessing the Meta Business support center, select Ad account disabled and log in with the locked account so that the system displays the correct form to submit.

Google doesnt publicly share a direct form for disabled accounts, but you can usually appeal through the Account disabled notice in your Google Ads dashboard. Click the link in the email or notification you received thats the official way to submit a review.

there will be no specific form, Meta's AI is smart enough to recognize and change tactics every day, you will not be able to bypass their regulations
only sometimes Meta has days that intentionally release so that you have a higher chance of successful appeal. I often appeal on these days
there are quite a few popular ways to appeal such as appealing to create a fb support department, messaging directly to the meta support team, or appealing through IG
these are all ways that can make facebook disable your account
