Client setup questions as a website?

Hi guys. I'm not sure where to post this so apologies if it's in the wrong section.

I'm looking for a more professional looking way of receiving a clients account credentials and some requirement info. I use PayPal for payment so I could link something to forward to after payment is made but I'm not sure what to use. I've seen some credential management websites but ideally I'd like to present them with a questionnaire.

Any ideas? :p

After. Asking for social media account credentials and their preference on certain marketing functions. Instead of sending them a word doc or questions in an email.
 
As a simple solution, you can use PayHip and redirect after purchase to Wufoo/Google forms/Whatever.

Do you have any coding knowledge? If so there are many solutions.

In that case if you don't find an option that is customisable enough for you, Hire a Freelancer section should work well :D
