The EPC on the first image doesn't seem bad, $1.15, although it depends, if it is from cold or warm traffic. The reason for the all order form impressions might be some competitor clicking. Use tracking to block ips in the traffic source, if this is the case. There are some affiliates that want to sabotage you, I've experienced that.
