Can't find a contact form

Hey Guys,
I'm looking now for a few hours for a contact form plug-in.
I want a contact form that users can send files like videos or pictures to me.

I hope somebody can help me out.

Contact7 doesnt work for me..
 
It is installing but to add the image/video upload thing isnt working..
Its a strange explanation i dont get it..
 
Common does somebody know a contact form that can send videos to my email or whatever..

You want an upload form, not a contact form. Here is a free one:

Just download the plugin which allows you to run normal code on your wordpress site and then copy the code into their and place it on your page. ;)
