Can someone help me create a 12question survey for my website?

I want to create a 12 question survey for my insurance project which we are doing in an area.
Can someone suggest me what is the best way to create a 12question survey including name,email address,address and other multiple choice questions.I want to do the thing which various companies do for the CPA offers by myself and I am going to promote them.
Suggest me what do I need to do for creating that and embedding them in my site without having to seek assistance from external websites.

Can someone provide any guide or material or suggestions as to how we can make the surveys by ourselves or provide a link to any survey making sites like how those CPA companies make?
