I am about to start a blog/vlog in the menshealth space.
My question is is it smart to go for a male only domain name or better to keep it broad in case I want to promote products for ladies at a later stage as well?
 
Better keep it broad for the future, don't limit yourself right at the beginning.
You could start writing and adding contents for males and then broaden up but limiting yourself from the get go is not a good idea.

Another thing I am wondering about: I will start the blog/vlog (youtube) in German but might later also add English content. Is it better to start off with a .ch/.at/.de domain or stick with .com if I later have the intention to add English content.
The question is also if it is advisable to have English and German content running on the same domain? What is better from an SEO perspective?

Do not mix languages make a sperate domain for each language much easier also from cultural point of view
 
Don't limit yourself right away; it's better to keep it broad for the long run, also don't mix both the topics content running on the same domain which may suffers your credibility and revenue.
 
I'd go with a broader domain name to give you more flexibility later on. Broad is better, in case anything happens you always have another option and if you see an oportunitiy to get more customer you will be able to take, is bette have different option to work on.

You can branch out into different areas of the niche if you fail to rank for some terms

More broad it is the better it is for you. Also do not expect to make any good money for first 1-2 years.
