To get BM5, make sure your current BM has good spend history, no violations, and aged assets. Adding trusted pages and verified domains helps.
 
I have 1 BM stuck at 3 accounts too. Spending daily for 1 year on 1 account, the other 2 cannot be used now. Is it possible to make it an upgrade if I only use 1 account?
 
You just need to increase spending, currently if you want to go up to BM5, your account spends about 100-200$ to go up to BM5.
 
Facebook only increases account limit from BM3 to BM5 for accounts that spend regularly every day and have no red payments

Should I create a page directly on BM?

I think you should spend small on the other 2 accounts and chat with support
