Imma_noob,
As blackjack mentioned, all you need to do is post your code into the HTML editing section of blogger. Make sure you take note of the URL after publishing if you don't want it linked to the rest of the site.

Why dont you host the opt-in form on Tumblr itself? Why put an additional step?
