Best US\Canadian proxies?

Hey guys,
I want to get like 5 private proxies for Instagram accounts on mass planner.
What is the best company with stable proxies located in the US\Canada?
Another question is do I need to refresh the proxies every month or do they last for long time?
Last question, what is the proxy username and password section on massplanner, do I enter my own?

2) You can choose to either keep the proxies each month or have them change.
3) When buying proxies, you have the option to enable login authentication. If you do, then you would create a username and pword, then enter the credentials into that section of MassPlanner.
