Best Affiliate Ad Plugin for WordPress?

I am looking for an affiliate ad plugin for WordPress that can, within an existing theme, and without much modification from the author:

1. Insert standard header ads (with a smaller horizontal banner),
2. Insert standard skyscraper ads (with a smaller vertical banner),
3. Insert sidebar ads (square),
4. Insert footer ads (with a smaller horizontal banner).

Any tips would be appreciated!!

You can use Ad Inserter WP plugin. It can insert any script/CSS/HTML to specific locations on your theme. Only you need bit of basic coding skills like creating html of your affiliate banners.
I use this for inserting Adsense code my sites.

For example, you create "Banner-1" and you can set rules like Display: "display on all posts", Location: "Top of page, First paragraph, Fifth paragraph, End of the post, After comments", Exclude: "Post-1, Post-2"
