What method does Facebook use to scan through videos for profanity or other things not allowed for ads? Im pretty sure humans arent watching all of the videos submitted for FB ads.

Also, is it true that if you regularly run FB ads without any red flags, they tend to ignore/miss red flags until someone reports you? I noticed that it takes longer to have an ad get approved if you've had issues in the past but the system loosens up after while. Anyone else notice this?

Yep, that's correct. As long as your process is good you can go undetected for a long long time. I ran a BH ad account for 3-4 months and spent more than 30k on it until they finally disabled the account after i started getting reported.
 
Not sure but I got an Ad for a Penis Enhancer on Facebook a few days ago. It had a White Pattern Overlay over it. Like checks over the video.
