Banned Twitter Accounts: Best method to get them back?

I understand that if you get your Twitter account banned, if you email them, a lot of the time you'll get it back, so long as you promise not to follow too heavily.

I've had one banned and I hadn't changed any follow settings for over a week. I was currently following 1554 people and I had 823 followers. I was following 145 daily, every 7-13 minutes. Quite surprised I was banned for that, to be honest!

Can anyone tell me the email/form to fill in, to try and get it back?

When you login on a computer a bar will show up at the top in read click the link to read more. There's a contact button in there in the drop down of the form select suspended account

I normally don't bother logging in manually, once an account is suspended.

I'm thinking of having a template email to send them if/when an account does get suspended - better than just deleting the account, accepting it's gone forever!

I still don't think following 145 people per day is that much though, to be honest.
