Back Link, but how to

So im just wondering, how do we "back link" sorry I know such a noob question.

What im refering to is the appearance of it. ( Sorry guys I cant post examples of what I mean, have noob restriction )

When actually backlinking, which is the preferred method, which should you avoid, or does it even matter?
