Auto Insurance Leads

Searched for some answers but all the threads are old so here goes. I have a car insurance site and it's generating 20 leads a day which average $15 each but I can't find anyone that can provide a zip code form, I'm getting these leads through banner clicks so I think there's room for improvement. I know of Vantage Media and Surehits but no one ever replies or answers their damn phones. Anyone work for someone or know of someone that can provide me a zip code form?
