Anyone using ukash/ukash out

Hi there. Is anyone here accepting payments via ukash voucher and cashing out by adding to a ukash out debit card?

What ID requirements are in place? Also, are there additional controls when a certain threshold is reached with that debit card (i.e. when X amount in total has been uploaded to that card)?
