Amazon stealth account in a new city

I was banned from Amazon about five years ago and have since only been selling on eBay / my own websites. I recently moved to a new city and want to use the opportunity to create a new Amazon seller account.

I now have a different address (completely different state), bank account, credit card, computer, router, and phone number. Is there anything I'm missing? I don't want to ruin this opportunity.
