Amazon Affiliate Sign Up Question

Hello,

I'm currently setting up an Amazon Affiliate website and I've read the ToS several times until now. I don't want to make a mistake when I'm signing up the Affiliate Program. I know that I need some good content first. But I still have the following questions:
  • Do I need a Disclosure-Page with an explanation what an affiliate link is?
  • Is it okay to put the Amazon Affilate disclaimer at the beginning of every page which contains at least one affiliate link?
  • After I got accepted by Amazon I can start replacing all the Amazon links with my ref links?

Most people put a disclaimer in footer so it shows on all pages

1) It's better to have a separate Disclosure/Disclaimer page saying that your site is a participant in the Amazon Services LLC Associates Program. You can have this said on your footer too. But I prefer separate page.

2) No, If you have a separate disclosure page, you don't need to add this.

3) Your ref link? You mean you want to cloak those affiliate links? NO! You can't do that. That's against Amazon's TOS.
 
Here is the content you need to put in a separate page.

"xxxx.com is a participant in the Amazon Services LLC Associates Program, an affiliate advertising program designed to provide a means for sites to earn advertising fees by advertising and linking to Amazon properties including, but not limited to, amazon.com, endless.com,myhabit.com, smallparts.com, oramazonwireless.com"

Its advised to have a disclaimer on each product page. Let me know if you have any other questions.

If this is the first time you register to amazon, you must use your amazon affiliate link from the start - Amazon will get your account full-approved only if you made your first sale by your website (You must list your site link on amazon affiliate account)

So I need to post the ref links before I apply the first time?

Just after you successfully signed up. Your account will be full-approved once you get one sale and amazon guy will review your website.

I've seen quite a few successful blogs with affiliate links that don't include disclaimer pages. Is this in violation of ToS?
