Affiliate Disclosure question

Hello BHW members,
I am a noob here, learning from the forum. I started my very first micro niche site - sort of personal-product-review site on blogger. I heard that I need to make a affiliate disclosure or google will ban the blogs - is it true?. Where do I place that?

And also, is there any standard templates you guys use? Do I have to make it personal too - like please buy from the affiliate link if you would like to support the site , stuff like that?

You should have a privacy policy on your site detailing the types of ads you use, such as google ads, and cookis policies and whatnot.

On my Amazon sites I also include an affiliate disclosure. I copied this from another guy on another forum who was happy to link me to it. Feel free to use it for you own use and change it accordingly:
