Hello!
Here is my share. I had a banned adwords account, every attempt at creating a new account was unsuccessful until just recently. I now have a working adwords account (thank god I need some money).
1. Get a windows VPS I use webkeepers they have a cheap one that is like Windows basic or something that is all you need.
2. You are allowed to access 2 things on this VPS from the moment it is set up. Adwords and a webmail account on a domain you have never promoted with adwords. NEVER NEVER NEVER NEVER go to any other sites on this VPS. Google has very sophisticated tracking and linking capabilities so WEBMAIL and ADWORDS only.
3. Change the computer name and workgroup on the VPS.
3. Call your bank and report your debit card stolen and get a new one, OR take out a new credit card. DO NOT use some BS virtual card. Google is banning those left and right. Never use any payment method you have ever used before for anything that has to do with Google.
4. Sign up for a new adwords account. Set up a new campaign pointed to a new site that you haven't promoted before. My best advise... set up a storefront shopping cart with OS commerce and some products. These very rarely violate TOS and are much less suspicious. Set up a very small campaign like you are a beginner. For your address use the numbers from your billing address and the Zip Code, but don't use your actual street. This way the AVS on the credit card passes, but your address isn't linked to the prior account.
5. Usually your new account will go under review. If Google asks you to contact them (which they did me) send the email FROM your VPS. Biggest mistake I made once was sending a response to Adwords from my computer that basically said thanks! They linked my IP from which I was sending the email and banned me.
Golden Rules:
1. New VPS with New IP
2. New credit card number and not virtual
3. Address number and zip must be real, street can be whatever.
4. Never access anything except webmail and adwords on VPS, NEVER INSTALL CHROME on VPS.
5. If you have to contact Google only do it on VPS.
