Adwords - How do you charge your customers

    Hey guys,
    Just curious, how do you guys who manage PPC campaigns for others do it all?

    Firstly, how do you lay it out?
    (1) Do you create a new adwords account for each customer (or have them do it) and then that customer enters their CC details in that adwords account?
    or
    (2) Do you have this under your own Adwords account and just charge your own CC and then bill the customer?
    or
    Something else?

    Secondly, how do you bill?
    The most common being a percentage of the spend (some charge setup fees, etc) . So if the customer wants to spend 1000$ p/m and your take is 20% - do you set the campaign limit to 800$ and bill the separate 200$?

    Have no issue doing this all for myself, but just trying to wrap my head around the aspects of doing it for others...
