I saw too many survey like this, is possible that some affiliation network give this?

Look around for CPAGrip, i think they allow adult content.

I don't really know who the exact provider is for that offer, but I have seen it across the net when checking various sites, so I suspect it is from an actual adult provider. It's not CrakRevenue but may easily be a similar network.

You could use http-live-headers and with a little research you could probably figure out which network runs it, or you could simply rip all the html content and adjust it to meet your needs on your own site. I like that lander. I may even rip it myself next time I see it somewhere. In the meantime, this site switched to running a different offer.

This is called a Prelander; a Prelander with qualifying questions, more precisely.

We offer many tours with qualifying questions just like this at CrakRevenue.

We may not don't offer the exact same one in your example per se, but we certainly have quite a few with those same type of questions.
