AdSense form not working (address verification)

After 4 unsuccessful attempts to receive my PIN, I got the option to upload the document through the form.

However, every time I try to fill in the form it fails and I get the
"Refresh this page and try again. Sorry, there was a problem with the form."

I've tried different browser, same thing. There's no other option to contact them.

Has anyone experienced this? Any advice is appreciated.

Try this, it can work:

Log out, clear cookies, temp and all etc. Use ccleaner or bleachbit

Log in from another browser and try again.

Otherwise give 24-48 hours gap. Could be temp block or something.
 
May be there is some error in your browsers. Try clearing history in all browsers (Select "All Time" in history clear option) and restart you computer. Then again clear the history like you did before restart and then check the form again. Also post this problem on Adsense Community Forum, I am sure Google Adsense Team has an eye on the Adsense Community Forum.

I've tried all of that. With different browsers, cleared all history, cookies, etc.

Same thing.

I'll try again in a day or two, but time is of essence here :D
 
Follow up on this:

I STILL have the same problem. I've tried everything, including trying from a completely different computer.

When I tried to fill in the same form using a different Google acc, it worked fine and I managed to submit it, but there's no reply coming to that one (I guess because the account from which it's submitted is not the same as the account connected to AdSense).

So now I'm days away from Adsense to stop showing my ads and there's nothing I can do.

maybe try with google chrome without plugins and accept all cookies?! worth a shot and nothing to lose

maybe sometimes waiting works. like in paypal, you need to wait to use funds even if they show as available lol.

Yeah, but it's been over a month since the form has been available to me and nothing changed. I'm hoping, but also starting to lose hope :D
