I'm getting 100K + errors in GWT with ?add-to-cart= urls.

I'm using Woocommerce.

These errors occur everywhere you find a buy button.

url.com/page/68/?add-to-cart=123456
url.com/product-category/ABC/?add-to-cart=123456
url.com/product/XYZ/?add-to-cart=123456

They show up under "indexing"

Alternate page with correct canonical tag
Page with redirection
Crawled - not currently indexed

Is this something I need to fix for optimal ranking?

It's common with Woo. GSC reports these because they're param URLs, but if canonicals point to main product pages and they’re not indexed — you're good.

Just as explained above, they're pretty normal to have as part of the website,
SEO-wise, you should block them via robots.txt to save crawl budget and avoid possible can, they're not meant to be crawled/indexed
