I have also encountered the same situation as you many times a day before approval. The reason is
1. page has many rejected articles
2. Ad account rejected
3. There are many offensive words in the article
 
Probably need to double check and likely make changes to your ad campaign. Not sure if the frequency of changes have any specific affect though, so best to keep track of that as well.
 
Next time, if your account doesnt spend any after long time active. You can check these belows factors:
1. The fan page (If it got banned/restricted from advertising or not)
2. The account got held or not (The pop-up notification).
3. The error from campaign (Delivery or others error).
 
Check first if the page is restricted also check if one of the admins of the page has a debt to Facebook
