BTW I've looked at all of the obvious causes such as cleaning cookies, useragent, email/password patterns, keypress timing/click tracking, etc.

I ordered a small amount from them and asked for FB ranges. On the first day I used about 10 of them to test and they worked beautifully. Now, a few days later I can't get anything to create even on the remaining proxies I hadn't ever used. I suppose as I type this it's much clearer to me that maybe I was on the right path but something changed with squid - like somebody else started abusing the same ranges I was on - at least I hope that's what happened. I'll acquire some new private proxies and post my results here.
