You need, in essence all new info. A checklist would be a new name, e-mail, address, phone number and checking account. You don't need to go as far as getting a new ISP. Using a different name and address isn't as hard as it sounds.
You can basically use any name or address you want. I recommend using an address you'll have access to. The name on your checking account does not matter as most banks will not share information with Paypal. As long as the account number and routing are legit it's good to go. Beware though some banks will not allow you to use another name with Paypal. I recommend Bank of America, Wells Fargo, HSBC, or Suntrust for stealth Paypals.
I also recommend making a new user account instead of clearing flash, it makes everything easier. A new IP address and a new user account is all you need to make a stealth account along with your new info. Good luck.
