Well, this kinda sucks

The CR sucks really bad, the clicks are good tho since it's 1/4th so that means that the landing page is working fine, but the leads? Not really.

just wanted to update. I get from 1-3 leads a/day. I guess i'll keep it as it is since it's all on auto so, i have other projects to take care of right now.

Just because the Landing Pages has a good CTR, doesn't mean it does it's job well.
Yeah, of course it's good that the CTR is decent, but the main purpose of a landing page is to presell the traffic.
By doing so, you will improve the CR, you can also try to direct link to offer, and see what preforms better.

Small bump!

This isn't going well, almost 1000 people clicked the survey but only 16 completed it, i think i need another method, suggestions? :s

Well you'll have to try the advertisers one by one.
Brazzers, CR etc etc, dating site offers from CPA companies.
0.02 EPC is a good rsult.
