FB has a pretty intense algorithm. If you just created a brand new account, there's a high likelihood that it won't spend anyways. Usually good farmed FB ad accounts take a couple of months worth of activity to really work. But if you're getting prohibited disapprovals, check their policy terms...

Facebook is a bidding based platform. The higher your ad CTR, the better the relevance score and positive feedback, the lower the CPC. However, you'll have a hard time getting $0.01 and $0.02 clicks unless you're targeting a 3rd world country.

You can site target with Google Display Network. And as a matter of fact, I recommend it. I've gotten way better campaign performance from GDN vs places like SiteScout.

Setup a unique campaign for Bing/AOL/Yahoo, then a separate one for syndicated partners. Also split up desktop/mobile/tablet into unique ad groups. I usually see accounts spend a bit better that way.

Name doesn't really matter. From my experience, I always make sure the billing zip matches the account location, but otherwise you're good to go.
