It's literally just the most likes/interaction to a comment. if you can think of something creative/funny depending on the original status/photo, people should naturally go to like it.

Interesting you should mention this, I read an article somewhere about Kendall or Kylie Jenner (can't remember who) being spotted with fake followers on twitter, so instagram wouldnt surprise me

Accounts that get a lot of rep seem to have access to it from what I've read. Don't complain, take advantage of it! I work for a media brand which has no verification and it's very frustrating. Although I've never actually looked for the form haha
