Ultimately it depends on your account age, and warmup. If it's not warmed for DMs you should take it slow do about 5 a day per account and up it by 5 every couple of days until about 20-30/day. (Not including responses you those don't count).

Ok, in general a good rule of thumb is to ditch the account if you see no good progress within 2 weeks. Make sure they are made properly on iOS with good phone numbers (no email that's weaker quality). Good quality ip addresses, unique.

This won't work on Android, because there's too many other identifiers. But it will work on iOS, just by uninstalling and reinstalling the app you will have fresh a installation ID which is enough.

I believe so, but I've also tried probably at least 25 providers who use dongles and spoof them, and had the same issue. Only alternative i ever found was mobile hosted ones.
