Since you add rel-author on your list. I think schema markup should be added also. If you can use rich snippets they have proven to increase click through by quite a bit.

I have often wondered this. There are still some niches that get that much search volume that it is still worth putting the money in for the traffic you get from Bing and Yahoo alone. If this is the case, who cares if you get penalised in Google. I do however only see this getting out of hand...

I found the anchor text distribution interesting. I did not expect the losers to have a high number of branded anchor text links. If anything I though this would be the opposite.

The only true white hat technique is content marketing. All other strategies involve some sort of link begging, persuation, or manipulation which will affect Google serps and they consider this unnatural.

Blog commenting does indeed work. the most important things to remember are try to high PR PAGES not just high pr sites and make sure the content is relevant. I always try to get the link in the actual comment rather than the author bio also.
