My theory is when you hit a certain % of deindexed backlinks you get punished by the algorithm. Not sure what that % is but try to keep it less than 15%. So yes building more backlinks from non deindexed domains should bring it back.

Changing the title tag usually makes my site (if it's new) go out of the rankings for 2-3 weeks. Don't do it unless you really have to.

I've had the same. That's because the site may have a "recent comments" widget in the sidebar. Therefore you would get a backlink on every page for as long as you're in the most recent comments.

You need to constantly update it I think. I have trouble keeping them indexed too. Maybe it's easier if you get involved with the community but that's too much effort for me.
