I'm going to do some research now and try to get this figured out! AutoWebLaw Pro seems to be the best option for covering all ground but it's a bit too pricy for me at the moment.

When I generated the policy it stated the programs that I'm affiliated with and that I have no control over how they use cookies. If this is enough to pass off as affiliate disclosure, I'm not really sure.

I don't think that's correct as long as you include an affiliate disclosure statement in the footer. I'm wondering if there's any software that available that creates the proper legal documentation.
