Yes you can get approved with squeezed page Yes you can open more than one account even with the same name as long as the info is correct in each account. Yes you can re-open this account or just open a new one

Well check under account go to Report > Balance details Maybe they have subtracted what you have earned for the previous month Otherwise you need to click on support center from your account and then click on ask question and ask them Good luck :)
