When/if you reinstall your OS its pretty important to zero it or completely wipe it as others have said. need to "zero it out" by writing it full of 0s. just selecting "delete partition" isn't enough, most malware today will remain after the repartition.

your problem is probably poisoning each ip through recycled user sessions, cookies, etc. make sure youre using a completely fresh instance every proxy
