1)Best way to test Instagram proxies is by using them for a while and checking how well they run.
2)I would say by running them like 2-3 fresh accounts per proxy and warming them up, if you don't have problems with the accounts in the first 1-2 weeks you're good.
3)Yes, I would rather lose fresh accounts then losing my aged high amount of followers account before testing.

You might be doing something wrong creating accounts.
You could always buy fresh accounts, they are really cheap and test them out.

5$ or 0.5$? I would say that if you buy less than 50 the price for quality proxies would go from 1.5$-2$ each.

The price does not determine the quality of the proxy.
When an account creation fails this is a sign Instagram already has some flags on this IP.
You should carefully test it to see if your accounts stay alive as long as with your usual IPs but personally I'd stay away since this can have effects on your accounts in the long run.
