2-put your email , facebook username.
upload your ID , if you want to upload fake one , try to upload certification (it has better chance to get approved) instead of id card.
3-press send and wait 10-15 minutes and you will get an email that they will look into your request.

I dont know when you tried it but about 3 weeks ago I tried with my personal account too.
