manually inserting ads

what determines whether or not you can??
i thought initially it was based on whether or not your account was verified, but i verified one of my newer accounts and i still can't add ads to the start and end of the video and also manually within the video.
does the video have to be long? if so, how long?

I believe they have to be at least 10 minutes long.

As far as i know there is no such feature given by you tube so far!

incorrect. go to the monetization tab on the edit video page and it appears there for 10min+ vids. you can choose to add them to the beginning (and end) which increased my ad revenue a lot on one of my channels. (4x)
you can also add them into particular times in the video and they will play at those times.
